TCER's work around chronic disease involves working with a wide range of program types (e.g. primary prevention, screening,  disease management) to make a positive impact on population health.  We work with programs that operate on a variety of levels and in a variety of settings.  Evaluation experts partner with organizations and agencies to help plan, implement, and evaluate programming in ways that both fit clients' needs and help achieve meaningful progress.  Examples of work we are currently or have recently engaged in are below:

Featured Project

Center staff have worked with the Iowa Department of Public Health 1305 Chronic Disease Partnership (CDC-funded State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors and Promote School Health (CDC-RFA-DP13-1305)) since 2013 to evaluate various areas of this program including; breast feeding promotion in Iowa hospitals, physical activity promotion in schools, team-based care initiatives in health systems and community-clinical linkages that benefit diabetes patients.  Evaluators have also provided evaluation capacity building services to IDPH staff through this grant.
